Your First Visit

Our objective is to make sure each of your visits are pleasant and informative. Your initial appointment will consist of a thorough orthodontic clinical examination. At the consultation appointment, each of us will do our very best to make sure you are seen on time, answer all of your questions and concerns, recommend treatment that we would recommend to our own families, and be absolutely flexible in our financial arrangements.

Please assist us by providing the following information at the time of your first visit:

• Completed online or printed patient registration forms and orthodontic health questionnaire.
• If you have medical or dental insurance, please bring your card or the necessary completed forms. This will save time and allow us to help you process any claims.

If for some reason you cannot make it to your appointment, please contact our office at (760) 758-0630. By doing so you allow another patient to use that time as well as reschedule your appointment. If it is after hours leave your name and number and a brief message on our answering machine and we will return your call in the morning.
